Jenny Sowers: My sweet friend was not given the care she needed. For her 3 month stay..she was given a crazy room mate that screamed and kept tv on all night.. staff told her they could not do anything to change it. She was forgotten too often for her dialysis. Food was mostly cold with no hope of seeing family in person after Covid restrictions. I talked with administrator, about my concerns... which caused nurses to treat her worse. DO NOT place anyone you love in this facility.
karen Nehl: The left hand doesn’t know what the right hand is doing, lack of communication from the top down, No one wants to take responsibility for anything, always passing the buck. Terrible follow through on anything in question. Please do not take anyone there.I will say that the oral, occupational and physical therapists are the best part of that place.

Jeannina Fraley: This facility actually deserves a zero. My sister was sent to Spring Valley for rehab following knee surgery on May 8. The staff disregarded any of our requests for medical attention. The facility’s doctor did not follow the orthopedic surgeon’s orders for rehab. Instead they only wanted to do speech therapy. Due to the lack of medical care, she became septic and she went into multiple organ failure. She was admitted to Mercy Hospital around 12:30 am on May 17 and she passed away at 9:30 PM. The facility doctor and social worker should not be allowed to work with anyone’s loved ones.
Anthony Ferrara: I give this facility spring valley health and rehab, four stars, only because they're a little understaffed, but every facility I've been in the last 2 years have been the same. The physical therapy is one of the best that I've had in the last 2 yearsThey have made all my requests that I have dealt with all my request, especially handling Medicare. The food is as good as it is suspected, the nursing staff has always been there when needed, the transportation department takes care of all my appointments, schedules them for me and make sure I'm on time to see the outside doctor's appointment, I also want to say that business office manager Teresa Gilpin has been extremely helpful in taking my welfare first and business secondI have been here 60 days Plus, And scheduled to leave this evening
Jamison Graham: So far this Facility had not impressed me one bit. My dad went for rehab, the Nurses are extremely rude. I walked into my dad covered in vomit head to toe, even all over the floor. Instead of changing his sheets they moved him to the foot of his bed, still covered in Vomit! The Nurses are rude when you call to get a report. Thwy dont even deserve the 1 star. If you put a loved on here, MAKE sure you are frequent visitors, to make sure they are getting taken care of
Rose Mooneyham: My brother just finished a 100 day stay at this facility. I had read the previous reviews and must disagree with the low ratings. He has really enjoyed his stay at this facility. He came in after surgery for a broken hip and his rehab therapists were very good at getting him backing moving and progressing in a short time. He found the nurses to be very caring and helpful. Like most organizations post-COVID, they are short staffed and maybe it takes a while to answer a call button. But they are working hard. When he first got there, the food was just okay. They hired a new cook and he's really bragged on the food. The nurses went out of their way to make sure he had what he needed. He wanted me to give it five stars but I compromised at 4. Rose
Melissa Perry: My FIL went to the hospital August 24th from Spring Valley, he passed away August 29th. My husband and I went to clean out his room at Spring Valley September 3rd which was ten days he had been gone from Spring Valley. His food tray was still in his room. There was old food all over the bedside table. His floor was so sticky that our feet stuck to the floor. Sad that no staff could find time in ten days to clean a tray and old food. Couldn't imagine ever putting anyone there or how they are still giving cares.

Came in the room and said what the hell.do you want I said I need to go to the bathroom Will I have 70 patients here to take care of They need to hire more help I was there for 10 days with covid. Cox hospital had no place for me so they sent me there.I don't know what Medicare paid.but was well over a thousand dollars Never allow a family too go therether
Aron Lee.: Never ever put your loved ones in this place. The way they treat people is awful! They forget to deliver food to your loved ones. When they remember the photo below is what gets delivered, on top of that the care " or lack there of" is horrible . They advise people to deficate them selfs because they don't have enough staff and they will come later to clean them up. They leave dirty used gloves laying all around the room, wrapers and pieces of trash lay around on the floor for days on end. They didn't deliver pain meds to patient for multiple days putting them through withdrawl. Please never put someone here.
Frederick Marlin: I was there 2 weeks for therapy. They wont let you in or out without a coworker!!! Its a prison!!! Never gave me my medicine. Dont go there. The therapy people are the only good people there. Went from 195 lbs to 165 in 4 weeks if I include the 2 week hospital stay before being transferred there.
Aaron Eagleburger: Couldn't even start the first full shift in dietary. I get there and find that the other two on my shift were out back. They came in ten minutes after to only go to the backroom to gossip withe the dishwasher who walked out the previous day. They commenced to talk about the person on that shift with said dishwasher. Saying how they ought to whoop her behind. Then tell me that I have to handle this guy with kid gloves. They were supposed to be showing me where to put the truck up. Everyone on their phones. Food is cold going out late. Grease Ll over the floor behind oven and griddle top. Food in both walk ins on the floor. Food in dry storage on the floor. I never got paid for the orientation nor the time on the first day. There are unlocked and unattended med carts in the hall I noticed on my way out.
Kristi Drinnin: My sister is here now. We want her out. No call light answered after hour and a half. Took us over 20 minutes for someone to answer the phone line. We were told we were not allowed to visit whenever we wanted. And a CNA told her she is not to call her family when she has problems. What? If it wasn't for residents in there no paycheck ks.

Betty Miller: My sister went into this rehab facility after her hospital stay.Tonight she was moved back to the hospital after being in this facility with Covid.She would wait hours to be checked on, she’s not mobile so she relied on them. She could NEVER get help after 10pm.She was admitted as triage in the hospital tonight so she went out in worse condition than she went in.The food was terrible!!I’m not how this facility is even open considering all the one star reviews.
Cheryl James: I had a close relative in this facility. They provided little to no care. The medication was not consistent in distribution. They did not change them when needed. The room was not cleaned as it should have been. The food was terrible and pretty much inedible. Their health deteriorated and they were re admitted to the hospitalThey also lied when asked questions and made it the patients fault and not their own inadequate training and care. One star is a stretch.
Minnie DoopDoop: My father is in this place and the staff is horrible, they will never let the family talk to him. They complain and say they cannot find a phone to take him. When we call they hang up on us and we talk to the manager and she was unconcerned with the way her staff treated us. Our father has severe dementia he even says they are rough with the patients. Don't send your loves ones here! Heartless staff
Rodger Roller: I'm Rodger Roller and I have too many bad experiences to mention. However, my last experience was they misdiagnosed a blood blockage for water on the knee. I left in my wheelchair and ended up in ER they put me on blood thinners as a result of Spring Valley neglect. I had a an infection in my big toe on my left foot and they amputated my left big toe as a result if Spring Valleys negligence. The food was cold most of the time.I will say most of the staff were decent. I do not recomend this facility to no one. Some people have good experiences and I didn't, to say the least. I'm seeking legal remedies as a result of their contumacious actions. It is my utmost desire to to seek compensatory n punitive damages.

Darlene Nicholas: After fracturing my ankle in three places, I was fortunate to be admitted to this facility. The staff is excellent and if you can’t go home directly, this is the place to go to help you in your recovery.

Jennifer: My cousin was recently at Mercy Rehab to recover from a stroke. The entire team taking care of her were phenomenal! I didn’t meet the PT team but her improvement from when she arrived to when she left was literally like night and day! One of her nurses I met,Cheryl, and her nurse assistant, Cat, are two of the kindest, professional and competent healthcare workers I’ve encountered. They both were so patient with my cousin and you could clearly feel the care and concern they had for her. They worked together as a team to keep my cousin safe and treated her with such respect. Everyone we met was smiling, polite and very much dedicated to their patients. I hope I never need their services but if I or my family ever do I’d want this facility to take care of us. Thank you all for what you do for patients.
Fred Wolford: The facility and the staff are outstanding. On a scale of 1 to 10, this facility is a 10. Dathan goes above and beyond. Karol, Brittany and all of the therapists are all above average. They are fun yet knowledgeable and professional.
Roberta Barber: I’mWe have been very pleased with our experience at Mercy Rehabilitation Hospital. Our Dr. Was Dr.Morrison who kept us informed about every aspect of the rehab. All of the therapist were very good, well educated and well trained. Our case manager was Renee Raynes who worked very hard to get us into the long term care unit we wanted. She had to jump through several hoops to make that happen and we are very grateful for all her efforts and keeping us informed every step of the way. I can’t say enough good things about our stay here.

Elijah House: One if my family members is in Mercy Rehabilitation Hospital right now and he is disrupted when he is trying to rest, and the door is always left open. Therapy is wonderful and they take care of him very well. Nurses and doctors are very disruptive in the morning when he is trying to sleep. They barge in the door at 6 and 6:30 telling him to get up and around when he just had brain surgery he does not want to be up that early. Me as a person that hasn't had surgery on the brain don't want to to wake up at that time so I can only imagine how he feels.
Johnny Cox: I just wanted to say I had one of the best experiences I could imagine at this Rehab Hospital. I went in with motorcycle injuries thinking the worst you know this isn't going to work you know they just look better than they are but every step they took contributed to me being better faster I was well enough within two weeks I was able to go home and when I went there I didn't think I'd be going home for at least a month. They're very helpful with everything . Homewise getting me back to my house everything they did was helpful all the way from the very top doctors to the people that swept the floors I couldn't imagine a better experience and I want to say thank you ,God bless you, and I'm so glad there's people like you around.
Melanie Woodward: It’s been wonderful and I’ve accomplished more than I thought I would and it’s changed my outlook on life literally. People have been very kind and I was treated very well. Leo had to help me with a shower and he was very respectful of my privacy. Caitlin was very caring when I was in so much pain. The PT student, he is going to make a great therapist. He builds you up and is by your side on everything. Rose, the CNA, is the sweetest young lady I’ve ever met and very caring. All the nurses are good. Dr. Neeley is outstanding and is a very caring man. He truly cares about his patients. I’ve just had a wonderful experience here.
Hilda Pinckney: I can't say enough about how great I was treated. From the moment I was rolled in until the moment I walked out. I was treated like a human and not a number. The care I was given was top notch. The nurses, therapist, housekeeping and kitchen staff all put the well-being of the patient first. Dr. Urman has the best bed side manor. He took the time to listen and address any questions I had upon discharge. They made sure I understood my medications and how to use my medical equipment. Pria (the rehab therapy dog) absolutely makes sure she shows you love and support. The PT and OT staff are among the best I've ever seen. I highly recommend this rehab center to anyone that needs any ţype of PT or OT therapy. I feel 100% confident that I will make a full recovery with the knowledge and lessons that I was taught at Mercy rehab.

Tasha Ganson: My dad was recently at Mercy Rehab for two weeks, following a complicated and extensive 47-day stay in the acute care hospital. When he admitted to MRH, he was requiring two people to get him out of bed and was not able to participate much in his own mobility. He was on a puréed diet and was still having some confusion from his lengthy stay in the ICU. The staff at MRH was excellent and skilled in caring for all of his needs. There were many people who participated in his care during his time there, but his primary therapy team of Eric (PT), April (OT), and Madison (SLP) all worked diligently and methodically to set goals for him and guided him through the process of achieving - and exceeding - all those goals. Madison upgraded his diet immediately after her initial assessment and it made a huge difference in his overall recovery getting to eat normal food again. His physician, Physiatrist Dr. Ronald Pak, was the most caring and compassionate physician - always knowledgeable and patient, taking time to truly care about not only my dad, but ensure my mom was also taken care of and informed at all times as well. Hospitalists Dr. Vuda and Dr. Brown also worked to manage all of his medical complexities and keep him moving forward. Nurses Tammy and Katelyn stood out (among many caring nurses), who made sure both of my parents were always comfortable, cared for, and informed - and always did so with a smile that showed their servant hearts. They did a phenomenal job ensuring my mom was trained in all areas she would need to be when taking him back home. There were many PCAs who helped during his stay as well, but Saul stands out as one who always went the extra mile to ensure everything was just right - especially in those early days when he needed so much physical assistance, Saul was always there for him. During the admission process, liaison Chaz Zumwalt was absolutely invaluable with her rehab knowledge, care and overall patient advocacy in working with the physicians, therapists and care managers at Mercy to get him where he needed to be. By the end of his two weeks there, he had become independent with nearly every measure and was able to return to their home after two months of being gone. I truly believe his time at Mercy Rehab may have saved his life. If he would have had to “rehab” at a lesser level of care, there is no doubt he would not have gotten all the extensive care and intense therapy he needed to get back on track… and I’m not sure he ever would have returned home. Upon discharge, case manger Jennifer was very thorough to set him up with home health care, equipment, supplies and anything else needed to be successful at home. We will be forever grateful to all the staff at MRH for making a truly life-changing difference in my dad’s life.
Joe Harkness: Ryan, PT at Mercy Rehab was my inspiration and all you guys have been incredible. He really loves his job and you can tell it especially when we know patients can be grouchy. All of the staff have been good to me. Tatum my nurse was another one that sticks in my mind from the night and Shannon from days.

Jeff Wilson: After 5 weeks in the Hospital with Covid 19, double pneumonia and 2 strokes I was sent to this wonderful rehabilitation hospital. The staff was awesome and Dr Bell was really the greatest. The food was top notch and the place was kept very clean. They worked very hard to get me out on Christmas day so I could be with my family.Thank you to the Dr's, Nurses, Food prep, the rehab staff and housekeeping. The food is beyond compare and nursing staff is almost perfect and PT and OT were far above expectations. The professionalism of the therapists is just shocking and over the top. Bailey is just beyond expectations. I. would highly recommend coming here for rehabilitation as its 5 plus stars.

John Lumb: I was here 15 days and aside from one bad encounter with an aide, the care was overwhelmingly great. Folks, don't expect to come here and not give 110% in Physical and Occupational Therapy. This is NOT a nursing home. If you desire healing, this is the place. I want to recognize the following team members for always stepping up even when they were busy with other patients.Sara...aide, Saul...aide, Becky...nurse, Cory...nurse, Dawn...nurse, Olivia...aide, and the girl aide with dark hair from the bootheel (Alyssa?) Scott Kinsey: I recently broke my leg and was sent to Mercy Rehab from the main hospital in Springfield. I spent about a week there, and in that time the improvement has been remarkable.During my stay, I tended to get bored in my room so I would wander the facility in my wheelchair for a change of scenery. I talked with many of the staff at the facility (from chaplain, to food service, to CEO, to nurses, aides, housekeeping, and therapists), and I was blown away at how wonderful they all are. This is an amazing group of people that are all focused and passionate on improving patients lives, and helping them get better / stronger.I will say it is not the most exciting place in the world to be. Each day consisted of 3 hours of therapy. Those hours aren't always easy, but the therapists were amazing at helping me build strength and confidence in areas I needed. We worked on things like core strength, controlled movement, and exercises I could do at home to continue to build that strength. The rest of the day is pretty much yours to do with what you want. I got bored easily, so I tended to wheel myself to different quiet areas to work or read. 21 hours in your room will get old, so I really recommend if possible you get out of your room.There is no way possible I can remember everyone that made my stay at Mercy rehab such a positive experience. Some of them were:PT"s: Rachael, Eric, Suzanna, & MonicaOT's: Ro, Beth, & AmyHK: Loeta (probably spelled wrong, but a sweeter lady you'll never meet)Aide: Raul, TiffanyNurse: Jill (maybe), PatAll the food service people were awesomeAgain, no way I can remember them all, but I would not hesitate to say that 99% of the people I met here were absolutely amazing. This hosptal is lucky to have such a fantastic team working for them there, and by the same token, Mercy must be doing something right to have attracted such a great group of folks.We all hope we don't need rehab for an injury, but if you ever do, this is the first choice in the area! Nursing facilities may offer similar programs, but if you're wanting to get better quicker, this is the best place in our area and I would recommend it to anyone!

The staff has welcomed me with open arms since day one and have also been very patient with me while learning new tasks every day and getting to know the residents. Our residents are sweet, fun and full of personality! The facility is beautiful, and I feel very blessed to have an administrator that I feel cares about not only my working needs, but personal ones as well.

This is the place to have your relatives stay in because they will be in great hands.
Christina Hawkins: I had a friend who was a resident of Birch Pointe. She absolutely LOVED the facility and the staff! Everyone cares for the residents as if they were their own family. My friend eventually passed away and the staff were just as heartbroken as we all were. I highly recommend Birch Pointe. Go in, grab something delicious from their bakery and check out this beautiful facility! You will not be disappointed!

Becky Hatley: First of all, let me say that in todays times, knowing full and well from a managers standpoint that it’s HARD to find good, qualified people who WANT to work… and working hard is what you have to do when working with those needing therapy or needing daily care in anyway, Birch Pointe has done a good job of it.We went from having our parents in the home with us to needing assistance 24/7 in the matter of a few months. This place is attentive to our needs. Bailey (social worker) & Amber (business office) have made the financial and social side of things so much easier. We initially moved one parent in and ended up needing to move the other in within months later. Both of these ladies worked hard to help us make this happen. When we lost one of the parents (age of 97) they were there to hug us and make sure again every need was met. Jon (GM) was great to work with in getting everything we needed to make the transitions as seamless as they could be. The parent that is left there currently, has severe Parkinson’s (and not the normal version of Parkinson’s either, the worst form of it). She is in great hands. Lindsay (Nurse supervisor) and Bethany (Nurse) along with Raven (aide) have been amazing. They answer our questions and make sure our needs are met and we have answers to our requests. *Now, has it all been sunshine and rainbows - NO, ABSOLUTELY NO!*. Nor was it when they were living with us. Have they received the care they needed, YES, and when we didn’t think they were receiving the care they should, did someone step up and find the answers, YES!! 100% of the time. I also recommend the LongTerm Hospice Team that they work with there. They have been amazing at stepping in while (like everywhere, including your local coffee shop or hospital) Birch Pointe is understaffed. I assume our world will look like this for a bit longer, and I’m happy to say with the friends we’ve made at BP I hope we stay in contact with a few of them even after we need their care.
Ernie Thomas: I was there for several months. The staff was amazing. They had shortage of staff like everyone else. They were compassion driven. Beautiful facility. The ot & pt did a great job getting getting me ready for my next step. The food for the most part was good. Occasionally it was a failure, any kitchen can. Jessica, Angela, Chelsea, Jeremy, kristina, angie plus many more I can't remember there names right now were great to be around & Truelycare. None of them were afraid to shift to diffrent roles if needed. I would go back if there services were ever needed again

Andrea Hultgren: My Mom had several stays at Birch Pointe 6/2022 and 10/2022 and I was so pleased with her care. Angela, Chelsea, Jessica, Cody, Amber and Bailey, and many other's who's names I can't remember, were all amazing and when my Mom left we missed them. Working in this field is difficult work and I was always amazed with how they responded to my Mom, even through they had so much work to do. I'm so grateful for all of you that work there and touched our lives. Your team brought comfort and joy in a very difficult season.
Chelsey Moore: I've worked here for 3 years now. My DON and ADON are amazing and always helps the staff when we are shortstaffed. My coworkers are caring and hard working. I always have help when I need it even though nurses are completely short staffed in every facility. Everyday we make the best out of our jobs. Our therapy department is amazing and I've seen such great improvement in so many of my patients.

Christen Hart: I had my dad stay at Birch Pointe for 10 days. He was treated with the most professional and caring staff. We have had some bad experiences in the past with other facilities but Birchepointe made it to where he and I felt comfortable with staff and the care they provided. If I had concerns about his health they were immediately addressed. My dad is a picky eater so when he refused to eat the dietary manager and nurses would go above and beyond to make what he liked. Which in most facilities we’ve been to in the past say they will but don’t accommodate. Which would cause him to lose weight he couldn’t afford too. The shower room was his favorite because of the whirlpool. He’s been wheelchair bound for 3 years so he got to take a bath and have the whirlpool massage his achy body.Upon walking inside they scan your temperature to stop any infection like the Coronavirus entering and infecting any of the residents. Btw it’s not a hand held digital thermometer. It made me have a peace of mind with my dad staying there. I also notice that the facility is very clean and up to date. The previous facilities we had stayed at where musty and run down needing a up grade like 20 years ago. Reception is very friendly and helpful when needing to know who or where to go for assistance. They also have a bistro where you can by beverages, food and even so yummy deserts. there is so much more I could say about this facility but all in all I’m glad I picked Birchepointe. We were very satisfied with the treatment and care we received. Thank you!
Rhonda Davisson: As a family we do not recommend your facility. Too many bad experiences to name them all. Never was able to get a hold of the nurses station by phone. Having one line is a problem no one ever calls you back. The ladies in the rehab department were very nice.
Debra Goodwin: My advice is never put your love ones in this place. My dad entered into Birchpointe in December he never had any water in his room at all. I brought it to their attention several times and nothing. He survived many health issues in his life to die after 6 months upon entering here. They reported to me 3 times that a staff member had Covid and on the last time an unmasked employee gave him Covid and he passed away from it in a week. I feel like they took my dad from me by being careless. We had to wear are mask and had our temperatures taken every time we visited him. These are people who are fragile and they should take every precaution possible when around them. I will never forgive them and I have to live with myself for putting him in there. So please, I would advise to not use this facility. DO NOT MAKE THE MISTAKE I DID that cost my dad his life.

Gail Gudell: My overall experience was very good. The physical and occupational therapists were amazing. They had a plan for me and I attempted to do every task…always fun and encouraging! The bedside buzzer was a problem…I had to wait (my longest wait for a response was 1 hour 45 minutes) the aides did their best but they seemed understaffed. I think putting a two-way speaker in would help immensely. That way you can get the right person on the first try!

JeffLinda Longwell: Very disappointing! As I am sitting here in the hospital on the cardiac floor with my mom due to their lack of attention and care, I am shocked at the treatment to patients. Call lights weren't checked on in a timely manner, beds were made with dirty linens, and nurses were allowed to talk mean and disrespectful to the residents. My mom had severe bruises appear from roughness. It's not a place I can recommend.
Mari Rech: A report has been filed with the State and hopefully this facility will be under legal investigation soon for elderly abuse. My Mother was neglected and abused. When management was made aware they assured the nurse was fired and the incident was being investigated. It was an "internal investigation" which was just a coverup.. DO NOT use this place. My mother's roommate said all the patients were afraid of the nurse that she said had "roughed" my Mom up leaving huge bruises.
Scott Kirch: The facility itself is top notch but what makes this place great is all of the people who work there. Both of my parents are there and are treated with dignity, love and respect. I visit frequently and am always impressed by the friendliness and kindness from all departments. From management to nursing to daily detail the hard work and humility shines through. Thank you for taking good care of my parents.

Lora Me: I was in this terrible place to recover, 24 days. Every person there was fed less than you would feed a 2 year old ...residents constantly begged for food. Some of the older ones lashed out at others to obtain a cracker! The staff is terrible... no one cares.... don't believe me? Just show up unannounced...the only good thing was the PT and OT....residents are seriously neglected and abused here!!!! Be aware
Katy Sunderman: I would have rated ZERO stars if that had been an option. The management is absolutely unprofessional and rude. The Director or Nursing is unhelpful in resolving any matter. Due to Covid restrictions and not being able to be inside with a loved one you count on the people inside. Unfortunately you can’t count on them at The Maples. Truly disheartening.
Roxanne Garnett: This place is beautiful yet management is HORRIBLE and willing to put your loved ones at RISK!!! Currently they have a high number of confirmed cases of covid-19 (including the staff) and little is being done about it. A former staff member tested positive for the virus and was told that they STILL had to work as long as they didn't have a fever.
Cynthia Johnson: I was in The Maples for about 3 weeks to get stronger. They are amazing. The therapists are great and so helpful and understanding. The lady that gives you he showers is amazing. The food is very good if you are on a regular diet. If you are out of the building when food is ordered they give you whatever. But most of the food was very good. Definitely better than hospital food. The aids are awesome. My only complaint is if you push the button for someone it can take a while. They have nice things to do to keep you busy, from bingo, to arts and crafts to getting your nails done. All in all not a bad place when you have to be away from home

While no facility is ever perfect, and every facility has its own problems, The Maples is the best facility I have ever worked at! From the simple things like having a good supply of personal care items for the residents/patients, to how management actually cares about the well-being of residents and family members. Then the larger things that we all hope for when finding a career or job, finding a place where management DOES care about the employees. The Maples is that place. Nate Tannehill: Staff has been incredibly rude and condescending to my grandmother. They have refused to give her the meds that she has been prescribed for years. Staff ignored her complaints of not getting her medications. Refused to call the doctor during the week to confirm her complaints and then tried to tell us "it's the weekend we cant get ahold of her doctor" although today is monday and a call could easily be made. When we tried to talk to the director of nursing she was extremely rude and simply told us we are wrong. I dont feel this place deserves even the one star I have given nor would I ever recommend this place anyone.
Kcteapot: Very bad time there. Didn't listen. On my Aunts medicine. Dont have rooms set up for patients needs. Unless u have a private room. The staff was very nonchalant about the issues. She was there 4 days. Worse when I took her out then when she went in.
Michelle West: I cannot say enough good things about The Maples. They took great care of my mother-in-law throughout her stay. The staff is friendly, professional and very caring. The facility is clean, updated and modern.
Jennifer Rule: My dad went to the maples for rehab after having a mild stroke. I know that most of the Cna's provided good personal care to my dad, but the administation/nursing team lacked in continuity. The Cna's were not cross trained to help build on what the therapy department had achieved. There were two Cna's that consistently took my dad to the bathroom using his walker, and this wasn't everyday. All others wheeled him in his wheelchair. We expressed that we wanted all staff to do this, but nothing changed. I would never recommend the maples to anyone.
Jennifer Sullivan: My dad has been in here for over a week and we couldn't be more happy with the progress he has made! It is very clean and laid out very well! We have had an issue with an evening nurse but I doubt she will be an issue again. Keep up the good work guys!!!

Linda Edwards: This place is abusive to residents and covers up physical abuse. They claim falls are reason for injuries but hospital doctors say injuries inconsistent with a fall and report them to elder abuse hotline. Lie to family members. Have been shut down numerous times for Covid outbreaks. Serious injuries to residents multiple times. This place should be shut down immediately.
Lacey Smith: This is a real review. I can't believe the 5 star reviews. Do not send a loved one to this nursing home. When you walk in, it looks very nice but DO NOT be deceived. They are unfriendly, the rules on visiting continually changed, my dad wore the same clothes ALL the time, the length of time they took to respond to my dad's needs was very lengthy as he listened to them have conversations/laugh in the hall, lunch would still be being served at 3:30 pm, all of the clothes we sent for my dad to wear had his name on them and disappeared within a week, his feeding tube was accidentally ripped out when an aide was lifting him out of his chair and they would allow his roommate to listen to his TV very loudly all night, every night. I'm sure the administrator purposefully does not leave her cards at the front desk due to the many complaints. This place neglects people. It should be shut down.
Rick: Read this before you make up your mind where to send a loved one. We had very little time to pick out a nursing facility and very little choices so we ended up with James River. Because our family member was in quarantine we could visit in her room. The condition of this facility is Not what you see in the reception area. The room was often very dirty and smelled awful. She was so miserable and complained often about the way she was treated. Until we repeatedly complained about her room conditions did it get cleaned. They were very short staffed and so conditions were not what they should have been. This facility is not what it seems.

teresa mccoy: It came time for my sweet little Mother to go into a nursing home after living with me for 3 years. It broke my heart to do it.But it’s only the third day and both my Mother and I are so pleased. Mom would rather be at home, of course. But she is adjusting very well so far. And a lot sooner than I expected. The first day/night was hard on her and I left there feeling like this wasn’t going to work. And that I was a horrible daughter.But the next day she was so much better.This started with my first call. The receptionist Sarah is so friendly on phone. She got me in touch with Bethany the social worker. What can I say? Bethany is simply the best. She answered all my questions and emails without complaint. And I email A LOT!!!!!!!!! She assures me that I can ask anything and as often as I like. She eased my worries and concerns from the get go.And Sarah was there at our first meeting too. She is so warm and made my Mom feel like they were best friends.Michael the administrator, is so nice! Not to mention, extremely easy on the eyes. Lol. He knows my Mom can’t hear well. He bends down to her level and talks right into her ear. It makes me want to cry when I think about it. I mean every time, he makes her feel special and cared for.Bethany, took care of all the little details to get Mom’s room ready. When George the maintenance guy was busy, Bethany was there fixing Mom’s bed before she was admitted.George mounted the tv for her. All with a smile.During meals everyone works as a team.Michael, Bethany and all the aids and kitchen staff are there.The staff are all so nice. Lydia, Jacob, Missy Sr and Missy Jr to name a few.Teri the activity asst, made a point to make my Mom feel a part of.Everyone calls her by name.The dining room is such a lovely place. And they have a great system in place.Lots of activities like bingo, karaoke and games.They get the residents together once a month for a round table of sorts to give suggestions and address concerns.They have a beauty shop for very good rates.Mom is a smoker. And they accommodate smokers big time. 6 breaks a day!!!! That’s unheard of.The residents are so nice and welcoming.And another nice thing is they let us customize Mom’s room. It’s very warm and inviting. Just wish it was a little bit bigger. But her power lift chair we brought from home fits in there, believe it or not.I am very picky. And I have asked for a lot.So for me to be this pleased is saying something.Ok nothing is perfect. But I get the impression that if there is a issue that they want to address it.Oh I almost forgot. The physician assistant or nurse practitioner, I forget what his title is. Anyway Doc Joe is great. He pulled me aside to gather as much information about Mom as he could. He filled me in on what he planned on doing for her regarding meds, bloodwork and so forth. He spent a lot of time with me.Okay, I am sure I have forgotten someone to boast about, but just know they are all so nice. I think the attitude starts with Michael. He has a fantastic team.Big kudos to them all!
Rachel Harrison: The nursing staff at Wilson s creek is wonderful. They have state of the art charting. The nursing home never smells like a nursing home because the staff is so kind and timely. As far as the other uglier reviews they don't know what they have til it's missing. The place I came from smelled terrible and was overran by cockroaches and spiders.
Elisabeth Misner: Egotistical, abusive management. Health is taken as a joke by the facility. 22 and COUNTING positive COVID patients, one of which has passed away. The management joked about "you can count on getting Covid while working here", after the information of a RESIDENT dying caused of Covid was relayed. Extremely morbid way of conveying the circumstances. The staff is laid back to the point of seeming to not care at all, they are there for a paycheck. Doing the bare minimum. Nurse didn't have a clue it seemed, given from the amount of pain she had put me through from not injecting the needle correctly for a TB test. I almost threw my arms up while the needle was still in me from the amount of pain I was in. If you speak up for yourself after being verbally abused, you can count on losing your position. The Administrator, Michael, condones in the abusive behavior of his staff. Very ill mannered, despicable place of work. God have mercy on the residents.
Elizabeth Morris Bryant: Today I had the opportunity to meet a very dear lady at the Wilson's Creek Nursing Center. I'm was my first time to the facility and my first time to meet my boyfriend' s Mother. The building was not fancy, it seemed clean with only a slight odor. we were immediately greeted and asked to sign the guest book. When we told the staff member Terry who we were there to see, she promptly began to make tell us individual facts about his mother. These included General Health, a brief overview of her activities for the past few days, how todays compared to recent days and very detailed info about today. We were taken to his mother's module, where April filled in details about her patient. Everyone was friendly and upbeat and very Professional. The staff was attentive as well as respectful. It is a hopeful atmosphere. The staff maintained HIPAA standards. The visit was Great! His Mom is a lady who has seen many things. She is kind and humble. It was such a relief to know the staff is watching closely and they reach out to their patients. This facility could use a bit of updating and that is what put the rating to a 4. Thank you Wilson's Creek! We will see you soon.
John Shirasaka: Place is horrible. Definitely one of the worst manage Health Systems facilities in the Springfield area. Administrator is useless and doesn’t care about the staff or any of the residents. Needs to be fired and replaced ASAP.
