Joe Rogers: I received a report from someone who is going through rehab at Quail Creek that a member of the staff was rude and hateful to her. I find this behavior unacceptable and unprofessional. An 82 year old in rehab should not be treated that way.
Toni Theimer: So, disappointing!!! My mother stayed in this facility for 5 days under respite care. When family visited her on Wednesday she was still wearing the clothes she had on Tuesday. When I arrived today at 4:30 to pick her up as scheduled she was still in bed in her nightgown wet. Family had requested the nurse at 2:00 pm to get her up and nobody had ever came back to assist or check on her. As she is unable to make requests on her own and requires oxygen that had no water in the canister and tube feeding that should have began at 4 pm!!!!!! They also, told me she would require a COVID test upon entering the building that did not occur. I would 100% avoid this placement if at all possible!!

chrys haws: OT & PT will have you back to being independent in your own home if you’ll work with them. They also have a great amount of activities to fill your day. The nursing staff is kind & attentive. The meals were awesome! If you didn’t like the meal they would cook you something else. I had a great experience.
Katie Heifner: We trusted my grandma in law to this care facility and I couldn’t be happier with the care she received. Everyone there was so kind and professional. She was nearing the end of her life and they took care of her exactly as she needed. The facility is clean, well managed, and had lots of activities. Plus birds in the atrium! It was so cool to pass by them when we visited her.
Wanda Anderson: My mom was treated with such respect and kindness! Therapist was were great. She was completely satisfied making improvements…however insurance dismissed her 😢. As we all know it always comes down to money. Who on social security can afford $281. a day?
Tom and Phaedra Ferguson: We are very pleased with the care, friendliness of staff, and cleanliness of the facility. Concierge labeled all his clothing to ensure we got his items back from laundry service--if clothes are taken directly to the patient room this critical step is missed. Dad's health and cognitive function improved noticeably after both of his 5 night respite stays at this facility. I was surprised to get bills from outside vendors, but it's a small price to pay in comparison to his 'nice little vacation' (Dads comment to me).
Josh P: Refused to take my mom because her medications are too expensive even though they accept her insurance and all medications are covered. Had doctor check medications list and they said there are none that would cause this issue. Won’t let her pay the difference out of pocket because they said that would be Medicare fraud. If this is what it’s like dealing with front office people imagine the nursing.

K Brobst: If I could, I would give QC 9 stars! From the Administration to the WONDERFUL Redbud wing Nursing Staff to the absolutely FABULOUS therapists the CNA's (Arlene in particular) and above all the Redbud wing "chefs" (Shout out to Kiana Cook) - this facility, in our experience is top notch. When admitted (after a 10 day hospital stay), mother was unable to walk more than 10 steps (with her walker) unable to dress herself nor use the bathroom. After 14 days with the AMAZING therapists, Jane, Tracy, Colton and Brandi (the four that I remember most) she was discharged to a long term care facility, able to independently physically take care of herself. The administration at QC was instrumental in helping us find a long term care facility that had availability for us with a memory unit. Thank you Shanda Bermudez (Social Services) from the bottom of my heart. Thank you Eric (Administrator) for having Judy (podiatry nurse) on staff to address those issues (I know that's not a position skilled facilities are required to staff). Jane (the memory book continues to be used, but confusion about the calendar and date is still a work in progress) Colton and Brandi (she practically races the halls with her "wheels") and Tracy, I can't thank you enough for all your support and "mental" therapy for my family. Naturally, no facility is flawless and we did have a MINOR, VERY MINOR issue, ie a buzzer that didn't function properly - so we got a bell, a TV that had remote issues (maintenance is still working on that one). I would HIGHLY recommend this skilled nursing facility. From top to bottom. BTW - if cleanliness is your concern - NO worries there either. Showers are given regularly and laundry is continual. Like I said...... 9 STAR
Susan Schoenthaler: Clothes and other property were stolen. Kept in a WHEELCHAIR to move around using feet, Except for 30 minutes of therapy once, maybe twice a day, Monday-Friday only!!!! Understaffed. Tylenol was given for severe pain. Controlled substances disappeared!!! 3 showers in 20 days!!!! Does not accommodate making food digestible. Pretty buildings hold substandard care and neglect. Please find another place for your loved one
Marie Sonnon: Non responsive staff who ignored call requests from clients. Left clients in restroom and did not return for an hour. Food was bad. Rude and very uncaring staff. Care was minimal. The PT staff was very good and caring. I can not recommend this place due to my own 💻 experience.
Cindy Ausilio: I was at Quail Creek to recover from and receive treatment for a pressure wound. My care was excellent. The staff was compassionate and skilled.
Janie Hartwell: I would never send anyone to this place. My mom was there for 18 days. She has been to two other facilities in the last few years and this one was by far the worst. They would leave her in the bathroom for a long time, her pills were all messed up, one time they said they didn't even have enough blankets for her to keep warm. She ate a lot of grilled cheese sandwiches. I asked if they give her a shower 3 days before she left and she never got one. I could go on and on but this place is not a place you want to send a loved one.
Pam Hershey: The staff were friendly and compassionate. The rehab people were great but the amount of physical rehab I feel was not given, probably due to my insurance. They had to do notes and charting as part of my rehab time so the hour of physical therapy was not an hour of everything physical. The occupational therapy in my case was not really needed but they had me doing it as well which could of been used for physical instead of seeing if I could wash and dry my clothes. Unfortunately in some instances not all of my dr instructions were done. Like he ordered a cream gir me and the nurses said he didn't. And also some other things gs he prescribed was not followed thru on. But the worse case was, the first full day I was there, while taking my shower someone got in my purse and stole something. When I reported it, the administration acted like it never happened. They approached me and said none of their staff would get into my purse. I had no visitors and no roommate. Not sure how they walked put by themselves.
April McDonough: I'm trying to think of positive things to mention, and there are a few. My mom went to QC for rehab after being hospitalized. The staff were pleasant and the aides were helpful for the most part. The food was fairly good, and I was able to visit whenever I wanted to. There were a lot more negatives, though. She was placed in a room with a bathroom that was so small she couldn't turn around with her walker. She fell twice in the bathroom, and we weren't notified the first time. She opened a serious wound on her leg. Communication was a problem too. We were not notified of her blood test results or the plan for PT and OT without repeated requests. We decided to take her home after 3 days, mainly because of safety issues. They neglected to tell us that the in-home rehab they ordered wouldn't accept her insurance, so we lost several days waiting to hear from someone. My dad was in rehab there several years ago, and it seemed much better then. I can understand some issues because of staffing shortages and covid protocols, but safety and communication are serious deficiencies at this facility. I would not recommend it.
Patty Goss: There are no easily accessible wheelchair bathrooms! Some of the staff was very kind. I told you who you were. Some of them were harried as they were usually short staffed.. some of them need to be brought up on abuse charges . (Don & his henchwomen that made the midnight rounds). The executive director was impressive. Filling in several nights. When I called before I went in I was told I'd get 2 hrs of therapy a day. Once I got there I was told no Saturday or Sunday. That took 6 days off of my 20 . No therapy on day in or day out now I'm down to 10 days of therapy. Only a few of the aides would allow me to pull my pants up & down in the bathroom. I was discharged after 17 days very scary when I got home .
Rhema Sanders: Worst nursing center ever. Hopes delayed on pain meds, they are low staff as of now, but their service sucks and there ability to pay attention to detail sucks.
Vicki Vessels: Loved rehab. The food was tough because I'm diabetic so I had to be a brat and ask for special things but they were kind. I liked all of the nurses rehab people. I had a couple of difficult roommates but I'm sure they thought I was difficult too. But no one's there for fun they're lovely people everybody just has a lot of problems as did I. I asked to go there and I feel like it was the right decision

tonya smith: Fancy place. Service was horrible to my mom! Aides talked about her like she wasn't in the room. They put her call light where she couldn't reach it so they wouldn't be bothered!! When she needed to lay down to rest they ignored her made her sit up 12 hours straight!! And now they want money. She can't communicate with you she is at Clio Cemetery. I will never recommend this place to anyone.signed her daughter.
Chris Lewis: Fort most part, good. But firewall kept me from watching my Church services online. There should be a provision for this necessary element of recovery!
jane wheeler: I am a 65 year old woman who had a reputured colon and stroke in August. After a 3 week stay in the hospital 2 in ICU, I got into Sunterra. I was in a weak condition, unable to walk or sit up due to a TIA . I was met right away by a nurse who assessed me, and when I mentioned I was hungry, a tray showed up within minutes. I arrived on a Friday around 12. Everyone was very pleasant and willing to help me anyway they could. The aids were fantastic, and the food was good with a couple of exceptions. I was on the 200 unit. PT and OT were wonderful. The aids were outstanding . The only downside was 3rd shift only had 2 nurses, and I had to wait for dressing changes . They do need more help on that shift, but they were doing their best, and I tried not to complain. My room was large and cleaned every day. I can not say enough good things about the staff of Sunterra.
David Estes: My 94 year old mother was admitted to Sunterra Springs after several weeks in the hospital. I had been told by several individuals that this was an exceptional facility. After my mother had been here for a few days it became apparent that everything I had been told was true. I quickly learned that no matter what your job title is, if you are employed at Sunterra, you are a care giver. The lady that cleans the room always has a word of encouragement for my mother. The cafeteria workers will complement her progress when they deliver her meals. The physical therapy staff is amazing. They have accomplished so much in such a short time. Each time my mother reaches a new milestone, they share the progress with me with so much enthusiasm. They truely care and it shows. The nursing staff is phenomenal. They provide exceptional care and have a genuine desire to help their patients in every way. And finally, administration, We have had numerous visits from the nursing director, human resource manager, cafeteria manager, social worker and the facility director herself, all for the purpose of making sure that every aspect of my mother's care was meeting or exceeding our expectations. This truely is an exceptional facility.
Daniel Bracker: FIVE STAR!!! Sunterra Springs (Springfield), Administration, Staff, Nurses and general personnel are absolutely THE best in the business.When you arrive at Sunterra Springs to visit you're greeted by Anita at the front desk. Ashley, Director of Nursing, and her staff of qualified nurses watch over and care for your loved one. Connie, the on-site Social Worker communicates and keeps the family informed on insurance and Medicare/Medicaid status. Every Occupational Therapist and Physical Therapist (Rae) work to develop and improve the patient in a kind and caring manner, while challenging them to improve. Kitchen staff were kind and delivered meals to the room on time and with a friendly smile. Lynn coordinated transportation among a miriad of other responsibilities at the facility. The facility was secure, clean and well maintained.My father's stays included a coordinated effort among every employee at Sunterra to ensure him the best care possible. Each member of Sunterras team were accommodating and responsive to all of his needs. I cannot think of one thing our family was not pleased about during both of his stays.If you're trying to decide on a nursing facility for your loved one; look no further. Hands down, Sunterra Springs Springfield is the best around.Thank you staff and management for the care you provided for my father. God Bless!
Tim Floyd: My mother took a pretty hard fall at home a few weeks back, and she ended up in the hospital, then a short term rehab stay to get some physical and occupational therapy. Sunterra was the facility of our choosing for the post hospital stay due to their 5 star rating, and their great reputation in our community. The process of getting my moms eligibility ran was very fast and smooth. We knew quickly that we had a solid plan for when she was discharged from the hospital. The Sunterra transport was there to pick her up as promised, right on time.Once she arrived she was very impressed with the appearance of the building both inside and out. Everything was well kept and very clean and modern. She was very impressed with the staff and their appearance and professionalism. She said that several employees came by to welcome her and introduce themselves. The room (all private rooms by the way) was very nice, very comfortable, smelled great and ......quiet. Her therapists were so good to push her to try to meet her max potential, but also show empathy for where she was at in her healing process. She also said the food was very good and not what you might expect in a skilled rehab facility. I would highly recommend Sunterra for anyone needing a short term rehab stay. Thank you to all the staff for everything you did for my mother!

The facility is extremely clean and the food that was served was top notch. I would recommend this facility to anyone that is needing a skilled nursing / rehab facility.

I have been with Sunterra for almost 3 years now and I'm very happy to be a part of such a wonderful interdisciplinary team.We have remained a 5 star across the board in our Medicare Ratings including staffing and quality. This last year we also received an award for being top 15% in nursing care from our Pinnacle reviews along with many other areas.If you are looking for placement for a loved one, I would highly recommend it. We are designed much differently as we are a small facility with only 38 beds and they are private only. We have a strong focus of rehab to home (reason I love it so much). We work with each patient individually to try to meet the goals of their care. We focus on high quality and are capable of taking care of a variety of needs.If you are looking for employment and would like to join please feel free to reach out to us and we would love to meet you and let you know what we are about. We will keep you on your toes with our admissions/discharges, but will give you a great nursing experience! We have a strong longevity of our staff and have built quite the "work" family.
Cindi Grimm: My dad was recently at Sunterra Springs. The place is new and looks fantastic. However, the care my dad received there was terrible. He lost almost 20 lbs in less than a month. He needed help eating and they promised they would help him eat. They did not! My dad got worse after being there. He also developed Covid (I don’t think he got it at Sunterra Springs) and was put in isolation for 10 days. And since no one could visit him, I honestly believe he was left alone most of the time. We chose this place because of the 5 star rating. Don’t ever base a facility on the 5 star rating. He had to be taken back to the hospital and spent 2 weeks in the hospital. He was then transferred to another skilled nursing facility that only had a 2 star rating. The place he’s at now, take GREAT care of him. Unfortunately we had to put him on hospice because he’s gone so far down hill that he can’t recover.The people at Sunterra Springs we’re RUDE, they acted like they were being put out if we asked them to do something that was within reason of what they should be doing. The day he was admitted to Sunterra Springs, it took almost 2 hours before someone came into his room to evaluate him and check him in. We told them everything he needed help with. They assured us that they would do all of these things. They did not do what they said they would do.They said he would be shaved, bathed, and hand fed. He was not shaved. My step mom has to do that. He wore the same clothes for days and finally after we asked when they would bathe him, they finally got him into a shower and bathed him. Their excuse was if he didn’t want to get in the shower they wouldn’t make him. Where he is at now, he’s bathed and his clothes are changed every other day. His hair is washed. They also shave him daily.I would not recommend Sunterra Springs to anyone. Now if, all you need is to be rehabilitated after a surgery and you can dress yourself and feed yourself, Sunterra springs may be just fine. But if you need more care and can’t do as much for yourself, Sunterra Springs is NOT for you or your loved one. They cannot rehabilitate you if you aren’t able to help yourself. Or should I say, they WONT help you unless you can take care of yourself. I only wish we would of taken my dad to a different facility when he first was released from the hospital and told he needed to be in skilled nursing.My dad was admitted to Sunterra Springs on July 11. He was taken back to the hospital twice and released back to Sunterra Springs. The last time he was taken back to the hospital was on August 11. He was at Sunterra Springs for one month and spent 2 weeks in the hospital. He is now at Republic rehab and Nursing home. They only had a 2 star rating. I would give them a 10 star rating. They are more caring and take so much better care of him. I only wish this is where we would of taken him on July 11. I think he might of recovered and gotten to a place he could do more for himself. Now, it’s too late. He’s 87 and his frail body just doesn’t have what it needs to fight anymore. He had Covid at Sunterra Springs and then also developed c-diff while at Sunterra Springs. Between the Covid, c-diff, and UTI’s, and being dehydrated… he just doesn’t have the strength to fight back. It’s all too much for his frail body.
Kathleen Dolen: Where should I begin…there is no structure as it relates to therapy. They just come and get you without any advanced notice. Showed up at meals or when I had visitors. I arrived on a Sunday. Did not see the doctor until Friday and an antibiotic was prescribed days before he saw me without an explanation of why. No plan of care shared. Inconsistent with dispensing medication, fail to tell you what they are giving, and had an experience where a med tech couldn’t tell me what a pill was. She said “I don’t know”. And LAUGHED. Told her it wasn’t funny. Had a situation when they were contacted on a Tuesday regarding an appointment time change for the next day. Didn’t tell me and I found out when transport showed up. They do not wear name tags nor do they introduce themselves. I had to ask names. Most of the staff if not very friendly or smile. I only know one name, Chery, a nurse who was friendly and took the time to explain what she was doing and wasn’t above checking vitals and helping with trays. Kudos to her!!! Food was not good. My husband brought me in a meal each day. Communication leaves much to be desired. Both my husband and son received calls regarding Patients that were positive for Covid. If it wasn’t for my family I wouldn’t have known. After I finally saw the doctor I requested discharge, I was that dissatisfied with care. This facility is not a “luxury” facility. It falls very short of that. I do not recommend.
Joseph Harris: My wife has received the best care at sun terra springs. I would highly recommend this place for your loved one. I could go home and relax knowing I didn’t have to worry about being cared for. My wife compared it to the Hilton inn but full service care. Thank you for everything!!! Janet Furr: Wonderful, caring nursing and staff. PT and OT work hard to improve your condition. Beautiful and clean facility. They are concerned about not only your physical well-being but also your social. They graciously provided coffee for a group of my friends to gather and visit me in the dining room and worked therapy around visits so I didn’t suffer from isolation. They also kindly transported me to medical appointments in safety so I felt secure.The veggie omelets are awesome, other breakfast items are good, and so is the pot roast and rolls. The rest of the food needs to be improved.They work hard to make your recovery quick and return you to your previous life, whatever that is.
Mike Tucker: I think this facility is nice, clean, and caring. The staff all answers questions and concerns, and tries to accommodate individual needs as best as possible. My Labrador was allowed to visit with proper paperwork, and that helped lighten the mood. I applaud people who take care of others for a profession. It's hard enough to do that with your own family :).

They are a dedicated, professional and very skilled team. The food, cleanliness of building and rooms, professionalism, nursing care and physical therapy are second to none. If you are looking for peace of mind as your loved one recovers, you’ll not find a better place.
Mike Eskew: This is a great place to work, and when you love where you work it really isn't work, is it? That is how everyone I work with feels, and it definitely creates happy, relaxed atmosphere. If you are able to stay here while getting better and stronger to go home, you will notice how comfortable and secure the environment makes you feel. If I could give it more stars I would!

The physical therapy is first rate and I think life saving. The facility is first rate. My dad was not eating and the dietician asked me what she could make that he would eat.You see administration and director of nursing interacting with patients alms the families.Thank you all!

C Spoerr: Not sure of other people's experiences, but a friend of mine had surgery and asked if I knew of a good place for therapy as it would be a few weeks. She wanted something close to her home in Springfield. Well with the help of a few "Angels, Christina & Kim", my friend was taken in at SunTerra. What a beautiful experience she had. She was so well cared for. The staff stopped by her room to make sure she had water, food anything she needed. She raved about how well she was cared for. Everyone was nice and spoke to her everyday. She just loved everyone there. Best place for rehab and the staff is ABSOLUTELY WONDERFUL! Thanks Kim for being on top of everything and making sure Carol was well cared for :)
Jean Brown: In early May I fell in my motel room and fractured my right femur. I ended up in Cox South hospital and after my surgery I had to have a place to go for rehab. I chose SunTerra Springs and couldn’t have been happier. I was not from Springfield but was visiting family there so I knew nothing about any facilities to go to for rehab and physical therapy. The facility is only three years old and I have never seen such a wonderful staff of CNA‘s and RN’s. I ended up being there 2 1/2 months and I honestly became friends with many of the staff. They were always kind, respectful and they treated me like I was in my on home. They honestly took care of my every need. They knocked on the door every single time they came in, even if I had used my call button. I had Physical and occupational therapy 5 days a week. The PT department was so good. I actually teared up the day I was dismissed to go home. I am doing so well and think it is a top notch facility. They have in wall oxygen, transportation to doctor’s appointments and many other great amenities. Please check in out if you, or a family member need such a place to recuperate.

Beth Morris: My brother is needing rehab. As a former dhss facility inspector, I must say I am impressed with the staff and personal support and attention offered. Highly recommend.
Becky Lipchik: I cannot say enough good things about this place. My husband was in here for a few weeks before he died and had excellent care. From the CNAs, the med techs, the nurses, the social worker, to the kitchen workers, they all showed him compassion and kindness and love. The food was excellent. Having experienced poor care at another facility prior to this, Springfield Rehab was a God-send for his final days and my family and I will be forever grateful.

Their facility is beautiful they make good food ha e good sized rooms and lots of activities for the residents they even have people come in and do nails for them and visiting hours seem to be whenever you want its just made me feel a lot better about my nana in this hard time we're going through thank you to the staff and everyone that works there!

The problem is the administration and head of nursing. We several times went in and found my grandma unresponsive and unable to wake her. We questioned her medication and found out they were giving her extremely high doses of medication that we had not been told of. She was so highly medicated she was having accidents in her pants because she couldn't get up to go to the bathroom. She ended up in the hospital from an UTI going septic. We shared our concerns with the hospital staff of things that we were aware of. They refused to send my grandma back to Springfield Rehab and found her another place to go. She has been there 3 weeks and we can tell a huge difference in her, she's much happier. The only reason this place deserves 1 star is because a few of the nurses and aides who, even though they didn't want to lose their jobs, shared concerns with us about how my grandma was being treated. My mom also witnessed other residents being yelled at which she took to administration.
N.C. Huffman: My 87 year old dear mother has 12 medicines she takes for a myriad of of health issues. Herarrival date at Springfield Rehabilitation was today. Her nurse, Mindy, spent an above and beyond amount of time, communicating and bonding with my Mother in helping to familiarize a medicine routine consistent withmanner her medicine is taken at home.I have been impressed with both facility and staff and the extra attention given to bothpatients and family. They try very hard toaccommodate in every way possible.
Beth Netzer: I would give less than 1 star if I could if I had a love one that needed a place to stay I would go any where but here there is no compassion for taking care of terminally ill patients not a place I would recommend. Since I first wrote the review it has changed alot of staff and with the new administration I would hesitate to check into this facility
Karen Dobson: When I have visited this community, I always get the feeling that the staff work well together, which is so important. We love working with Amy the Social Worker, she is very good to help with any thing and is a great advocate for the residents.
Scott Duncan: I've been in and out of this facility for various reasons in the past 19 years and can attest to the fact that this is the best care you could possibly find in Springfield, Mo. Becky Solari: My dad has been at the rehab for 2 months and is going home this weekend. The physical therapists here are wonderful.. full of energy and such happy attitudes! They worked my dad hard and kept him motivated to get his strength back and able to go home:). There are some very compassionate and skilled aids here .. Jay, Michelle, and Victor have become like friends to my dad and our family. They are all stretched to their limits, as I feel is the same in most skilled facilities, but they do their best to keep up with the patients needs. I felt the nursing staff was competent, sometimes you had to remind them to check on your loved one, but again, I feel like they are stretched to their limits. We made many good friends with the staff while my dad was a patient here.. the management needs to see what gems they have in their staff and hire more faculty to help with patient care! Our social worker was extremely helpful in getting things started and with helping with home health and physical therapy set up when we were dismissed. I highly recommend Springfield rehab .

Jessie McDonald: I have worked for Jordan Creek for over 16 years now. Over the years we have gone through many changes. However, there are a large group of people that have been here a long time. They stay for two reasons love for the residents and love for the building. While some residents and family may not be happy with our care. A lot of the residents are. You can see this when you walk through the doors and see their smiles. So, before you make your judgment that was just another nursing home. Please come see what Jordan Creek is all about.
Finding missing loved ones: The only reason I am giving this place 1 star is that is the only I can place a post. This is one of the worse places I have ever seen. There are maybe a handful of good CNA’s at this place my Aunt was Here for rehabilitation and the short time she was there somehow her kneecap got broken because of the way The staff handled her. she ended up with a leg brace. The physical therapist stated there is to be no pressure put on her leg. The staff was also told that there was to be no pressure put on her leg. When the staff would come in to help her use the bathroom they would not use about 90% of the time and pressure would be put on her leg after a weeks time her feet started to swell and they would not call 911 my aunt ended up calling herself. The reason for her foot swelling was because she was putting pressure on her leg to get herself to the commode because when she would push the call button for how she would end up waiting anywhere between 20 minutes to an hour for someone to come to the room towards the end no one would come to the room. Kay is one of the CNA’s that would not help my aunt at all. Kay laugh in my aunts face when my aunt would tell her how much pain she was in. My aunt would ask for her pain medication and they would not come back with it until 3 to 4 hours later. My aunt ended up in a different rehabilitation center and is receive bettercare and is expected to go home in about two weeks.In addition food is awful my aunt does not eat beef or pork and they would still bring her the meal containing these meats. Even if she ordered something else they would bring her the wrong tray

I'll be glad to tour the building with you.♡☆This is an incredible place to work.☆♡I have repeatedly been offered opportunity and have been treated very well here by the staff. Our residents are A-MAZING. And they are very loved and cared for by our staff. If in doubt, come ask the residents themselves! I understand not everyone will have the same opinion as I do. All facilities have ups and downs and over come things together. I have found, that Jordan Creek does the very best it can to work together through what ever comes. And that counts for a lot to me!

Ashley Balentine: Just awful. These poor residents. HSI is a large corporation and their nursing homes make that clear. Any way they can save a dollar, they'll do it...which most frequently means the residents suffer. Please please please look at all of your options before placing your loved ones in a skilled nursing facility. Look at the residents' faces, fingernails, and shoulders. When you see flakes of skin falling off of them, know this is how your loved one will be treated, as well. Look at their fingernails and estimate how long it's been since their nails have been cut....and know that's how your loved ones will be treated. Go to the Baptist Home in Ozark...it's the best I've seen. I could tell you real horror stories about HSI but I don't have enough room. Please, do your research before placing your loved ones in a home. They are the last great generation and deserve so much more than HSI is willing to give.
Alexus Tindle: Overworks their workers, doesn’t treat workers with respect and underpays their hardest workers! Allows staff to treat other staff horrible and doesn’t do a thing about it! I would go into more details but I just wouldn’t want their company to go under! Which is should! Horrible place to have your relatives if this is how they treat other people.

Wright: I've worked at jc.now for 14 mons.now.like every other nursing home some days are just better than others.lately though I feel as if the environment as a whole is changing for the better.everyone in that building though needs to work together.being an aide though truly is one of the easiest jobs I've had.honestly I feel there is a lot more that admin.could be doing to improve at least our work place.and yes I know it's not all about us,but about the residents.yet if you can't care for yourself your team your employees then how can you take care of anyone else?I don't have all the answers,I've got some ideas,sure.we can only hope our new d.o.n. And a.d.o.n. will stay longer than 6 months.i think that would be a plus because we need strong people in those spots but for longer than a few months.the changes for the better may not even be seen by us,but it's awesome and rewarding to everyone that takes part in at least making some kind of effort for our people to live better.i feel at meetings we should address more serious issues.staffing,the petty drama,And all other "life situations"we all know the issues going on yet nothing's addressed.human kindness,caring for others residents,co_workers,just other human beings in general is a natural human trait it takes no effort.all the other shim-sham is actually a lot more effort since you are going out of your way to do it.we have the opportunity to help our community and fellow man and woman with the residents.the staff here are all amazing people as well and we together can make jordan creek a very nice place to work and for our residents to live.not enough is ever said about our therapy crew.good example of team work and enjoying the job it shows in the progress they make with our residents.everyone of us that work in this biz.help people everyday that's a good thing so find your enjoyment in that somewhere.cause no one else will pat you on the back or elevate your ego any or even thank you for your hard days work,but that's ok.some days are just better than others...in the big picture of nursing though just know that you are important to our residents,our company and well the human race altogether.and you may not truly be rewarded until your looking up at your aide one day.and at the very least you could always just be professional for your shift.i said all that to say this,bring the shout out board back to jc.

Personal items are stolen first night he were there, he slept in a wheel chair first night there at the nurses station . He needed special dietary diet and was given same food as everyone. Took him out the next full day.. i wouldn't even take my animal there.
Abby Akin: I know this place does not look as fancy as some of the other places but the staff was so wonderful for us. They were very proactive and cared very deeply for my mom and our whole family. I toured every place in town. Literally, I toured EVERY SINGLE PLACE in town and talked to the staff at each place, asking them detailed questions. I spent weeks doing research. This one was better than any other option for our needs. I appreciate the staff there more than they will ever know.
Mary Atchison: If I could give this place a minus star or 5, I would. My father had a heart valve replacement. When he left the hospital he was walking, eating, and talking. Five days at this hell hole, and he was almost dead. We had his doctor go over and check on him, since we live 300 miles away. They had a fit and sent him back to the hospital. He had pneumonia in both lungs, a kidney infection, and bad infection in his groin in the incision for the replacement. A month later, he can no longer walk, he is out of his mind and babbles. We are looking for long term care, and I promise you, it will not be in this place, and hopefully not with the company that owns this nursing home. If you love your loved one, please keep them out of this place. By the way, someone took his clothes. The nursing home went to walmart and bought him new clothes. Unreal.
Karin Widebrook: Worst place on earth. My father went here on a Wednesday and was still walking. On Friday morning early am he was apparently fighting with someone who stole his watch. He had been cut on his arm and bleeding. He had a stroke and passed away on Sunday! He was in the same clothes he left home in and was never bathed or given his medicine. A family member was there the first night he arrived and he was happy knowing he would only be there until Sunday, when he would be returning home. He only went there so my mother could get over her flu. He didn't have water and my daughter asked for it for him and was told he didn't want it! Same thing happened the next day on her visit. I went to collect his belongings on Friday and couldn't find any nurses. The man at the desk was the bus driver for the home. It was terrible when I found his teeth on a nightstand across the room. A picture my daughter took in Thursday evening had been broken also. I know in my heart he would have had him longer. I'll never forgive myself for letting him go to that place.

Marlana Kimmis: The administrator at this location is absolutely a God sent man! He took the time when I was upset and stressed over my mom to sit and talk with me about what my momma needed and the care she got while in rehab. He never once place judgement on me or other places she has been. He just helped me!!! That is a lot in this day and age for someone to treat you like a person and not just a number. I hope every one has a chance to talk with him and his staff because they aren't just there for the money they are there for the patients!!!!!
JeffLinda Longwell: Placing your loved one in a skilled nursing facility for rehabilitation can be a very scary thing. After much research and consideration my family chose Glendale Gardens. The staff was kind, considerate, and very caring. My mom was in there three weeks and we were very pleased with the facility, her progress in there rehabilitation program, and the professional staff.
Child of God: Horrible place! Bed bugs! Don't keep it clean! Can't keep staff! Feel sorry for the few good workers there! Takes forever to get someone when they push the call button!
Laura L. Quinn: This is NOT a skilled nursing or rehab facility! It is a warehouse--BEWARE!! On my mom's first night--after 11 days in the hospital--she had a medical emergency (bleeding) and waited 4 hours before anyone responded! A nice CNA did respond, but left to go "find" a nurse. No nurse for another hour...then a text to the "Doctor on call"...no response for another hour...(and, ultimately no reply was forthcoming). So my mom called me at 5:45 AM out of desperation. We called the hospital where she had just been discharged and they said "get to the ER now, by ambulance or car!" I told a staff member, who replied that they would do nothing until the doctor texted back ("sometime today"). I found a wheelchair in the hall, got her in, we rolled out to the car and we made it to the ER in time for emergency treatment. No one at this facility attempted to help or has ever contacted us to follow up. So-called short staffing is one thing---utter disregard for a patient is quite another. TOTALLY UNACCEPTABLE TREATMENT!! If you value life, STAY AWAY!!!

Maddie Mela: Glendale Gardens Killed my Grandpa, We busted him out of the facility on a Tuesday after saying he was neglected and thought he was going to die there. He was dead Friday night. Horrible facially. Do NOT let Hospitals or yourself send your loved ones here.

Valerie Gardner: Our sister Vereen battled cancer. We needed to make the hard decision to where we had to place he in a facility to give her the end of life care. This was honestly one of the hardest decisions we ever needed to make especially with the Covid restrictions. The director informed me that when the time came, we could go in to be with her so she wouldn't be alone.I really needed to make sure after she passed to let all those people who are making this hard decision that we needed to do, to feel that Glendale Gardens "IS" the right choice for your loved ones. The staff of nurses there are top notch and the have the BIGGEST hearts to care for not only your loved one who will be there in their care, but also to help you in that time needed to say goodbye.I just want to thank the staff of Glendale Gardens for EVERYTHING they were able to do for Vereen and the family who was left behind in this hard time.You are all amazing and truly our heroes.Thank You
Dale Hampton: Just give them a call. That will let you know how horrible this place is. Cause they dont answer. Either side of this place is just plain sad. They do not care for the people they have there. They do not help rehabilitate people. They make it impossible for someone to recover from anything. They throw them in a bed. And leave them. Its truely a SAD and HORRIBLE place for anyone to go. Wish i could give zero stars!!!
