Monterey Park Rehabilitation & Health Care Center: what do users think?
Bradley Gene: I recently left a bad review on here. But after several days, it has gotten significantly better. The issues I was having was due to one particular individual but has since been resolved. Of course every facility has it's flaws, but the overall care has been much better than when I first arrived, therefore I had to update my review to a more positive one.
Oma Oma: Not impressed. Care was very nonchalant, in two weeks, bed linens never touched or changed. Food was terrible, missing items (served butter or jelly with no bread), not diet appropriate. Discharge was not informative, sent home insulin without instruction or needles. Patient had never took insulin at home prior. I could continue but they need help getting it together. Staff was friendly most of the time although took a couple hours to see anyone if nurse button was pushed. Overall disappointing.
Morgan Slavens: Excellent company to work for, they have corporate knowledge & know costumer service for employees & residents along with having the nursing knowledge to run a successful nursing facility.
Cynthia Goode: I enjoy working the rehab unit.
ric delaney: I was placed here from my hospital as they told me this was a good spot to heal with skilled nursing. They were right. It’s clean and quite responsive to my needs as I am bedridden and a bit helpless (for now). The director/manager sees me each day as do therapists of all totally professional fields. Everyone I have met has been kind and compassionate and caring. I feel like I’m in good caring hands. I trust them to get me back on my feet and back in the game of life. I watch them taking care of the elderly and have seen firsthand the care given these somewhat infirmed folks. I would describe it as respect and gentleness. The room is the clean and large and the cable worksLike any facility of this type the food is always in the forfront of anyone staying here. But it is healthy and timely albeit the manager said she is always searching for improvement. That’s an attitude I appreciate. I have been seen by the sharp resident doctor and her nurse practioSo here I am for a few weeks. I’ll keep you posted. Ric delaney room 505.
Kris Cantrell: Monterey Park has a dedicated staff committed to serve the residents with respect, dignity and integrity.
Julie Tuione: I would give it zero stars but I can’t! This place is straight up elderly abuse! Ive never seen anything like it! They are super understaffed . And you have to ask them for your meds each time or they don’t give it to you! They didn’t disclose that they had a Covid outbreak before my mother was transferred there, not until she was literally in her room. Now my mother has tested Covid positive from their facility. She was tested the first day and was negative. It came from this facility who has an entire hallway plastic covered to the Covid patients but none of this was mentioned for us to make a decision to not move her In there. Total neglect ! Then we moved her to the hospital because of her Covid because they don’t treat their patients for Covid in there ,! What a surprise! And they wouldn’t even bring her belongings to the front . Here is a health facility spreading Covid even more by not bringing the stuff to the front and having you enter back into the facility. Way to go! Never again! I wouldn’t recommend to bring your worst enemy in there! Not fit for a dog! DON’T DO IT!
Jon Meyer: I wouldn't recommend Monterey Park to anyone. If they are short staffed quit adding people for CARE then. If anyone does come to the room it seems to be putting them out. If you hire people, hire someone willing to work for reasons other than for pay checks. I have only met one employee with the attitude of wanting to help. The young lady in physical therapy is only one I can trust so far in this place. I really don't feel like I am safe or going to move forward in my plan to return home again. I think this place has one policy. That's to make people have to stay inside of the place as long as possible, if not for the rest of their lives 😡😭😡😭😡😭😡
Elisa Smith: I just moved my Mom out, should have done it faster. Staff turn over daily, no one answers phones at night. Ancient medical equipment
Brigitt Arndt: CNAs have never asked my dad if he needs a shower. They are supposed to every morning. Joshay doesn't hide that she doesn't like her job. She has an aggressive way of cleaning my dad! It takes over 30 minutes for the real nurse to be told by CNA that meds are needed. Cleaning only happens by request if that. If I need these services I will look elsewhere! Update: Nurse Kelly called in cuz staff issues. Nurse Joe quit cuz no CNAs showing up! Everyone in my dad's hall got Covid from this place! Get it together!
Susie Green: I was there for a couple days released from hospital wouldn’t give medication almost killed me recieced know asstince had to call 911 to return to hospital
Debbie Bruckmeier: Do not ever allow your love one to go here. My mom was there for rehab. I have a 7 page list of all the horrible things they did and didn't do. The worst is letting her sit in her urine for who knows how long. She her clothes and bed soaked. I would go visit her for about 3 hours every evening and I would have to change her. I could go on and on. WORST PLACE EVER. I feel so sorry for the people that are still there.
Rebecca Brennan: Monterey Park has a strong background in the community. Staff have many years of service. I enjoy working with the wonderful group that care a great deal for the elderly. They constantly strive to improve all residents life’s.
Hal Davis: Mom often shares about how nice and helpful various staff are
Marla Kimsey: The staff at Monterey are the most pleasant and kind associates. They are at times shorthanded but are always working hard and dedicated to the patients. Mom has always had clean sheets and bedding. She does not like much of the food that is served. They have a daily paper that is really great.
Amber Williams: This is a wonderful place to work!
Marina: This place has always been at the bottom of a pit now more then ever
Karen pettes: My sister has been here twice ,both times she has received wonderful care , the rooms are big , staff always nice .Would recommended Monterey park
PaperHearts: DO NOT SEND YOUR LOVED ONES HERE!! I used to work here and this place is the most disgusting place I have ever worked at. People are left unshowered and sitting in their own feces. The administrators do not care about anything but a paycheck. Workers vape in front of residents/next to them. This place should be shut down.
Tiffany Hammons: If I could give this 0 stars I would. This place is horrible! My 97 year old mother in law has a broken hip and a night nurse told her to get-up-and-go to the bathroom by herself. The head staff and front desk are rude and harassing, yelling at people for masks when their staff don't wear masks or they have them below their chin. What a joke. They yelled at me because I was taking a drink out of the drinking fountain and told me to pull my mask up. Really how do you take a drink with a mask on? Just really hateful ignorant people working there. I wouldn't send my dog there.
Debbie Hatfield: communication b/t staff apparently doesn't happen; what you are assured on coming in--we'll dress her daily, for example--proved not to be true; half the time if her bedclothes have been removed, clean bedclothes are not forthcoming so she naps without a sheet on the bed or a pillowcase on the pillow unless a family member is there to force the issue; many staff seem well-intentioned and a few actually are Earth-angels, but overall the staff appear stretched too thin to supply my loved one with the look-in she needs every hour or so. Food ain't bad, though.

Sunterra Springs Independence: what do users think?
Connie: Sunterra Springs is poorly managed and ran by contract labor. DO NOT send your family members there as they may not receive proper care. My family member was SENT HOME WITHOUT a walker required to get around, as well as, no oxygen or medications. If my family member did not have family to advocate for them it could have resulted in physical harm.
Ashley Leeson: If I could give this place zero stars I would! My grandmother went into this place using a walker, and will be leaving in a wheelchair! No explanation why. The social worker has not been any help. My mother, and aunt have reached out several times for an update, left messages and no response! Staff, from the assisted living she will be going to, also tried reaching out several times needing paperwork and medication list. This information was needed prior to her moving in and after several calls/messages they received information late! My mother and I came to visit, and my aunt who lives close by came to visit several times and not once did anyone come in to work with her. This place is awful, I do not recommend to anyone!
Mary McConnell: So far so good. I haven’t started any therapy yet so I can’t really say how my treatments going. Staff very helpful and friendly. The facility also has good food.
Kim Clark: This isn't a place you would want to take a loved one with cognitive impairment. Lots of agency staff being used. And the Social worker doesn't really help family members with post rehab arrangements.
Nicholas Sean McMillan: The have some of the worst cna's. Some of them take at least 30 minutes just to help. Two times they left me on a bed pan for over an hour. They are rude. Stay at your own risk.
sarah dimaso: This place is beautiful, and for us it was conveniently located. CAUTION: if you want to visit your family member or friend while they recover, maybe choose another facility. Visitation hours are impossible for someone who works during the day and the visitation policies changed 2-3 times in 3 weeks.
betty kindler: My mom came home today after a 37 day stay at Sunterra springs. She had the very best care! I cannot name one single staff person that wasn’t helpful, polite and caring. She is 94 1/2 and broke her femur. The physical therapy, occupational therapy and speech therapy was all top notch. When she arrived she could only take three steps and was in extreme pain. Today she walked out with her walker, hugging everyone and crying because she’s going to miss them. She said, “The one positive thing about my break is that I got to meet all of you.“ THANK YOU to everyone on staff!
Robyn Rhea: I just visited my Aunt Ruby everyone was very friendly
Harold Edwards: My Mom came home yesterday from this place. She had a fractured vertebra and was ordered to have bed rest for several weeks. We struggled to find a place for her to go as we could not take care of her at home. She was sent here which looks like a very nice facility, new and everything. It's really hard at this time because they cannot have any visitors. The only contact we had with her was by telephone. They would constantly leave the telephone out of her reach until we complained. The nurses for the most part were nice and helpful. Some of them were gruff and grouchy and unproffesional. One time Mom needed to have her diaper changed, the people who came in to do that were very rough in moving her around. Then when they asked if she needed anything else, they told her they were trying to watch a television show.An hour or so later she asked for help to shift positions and they again were very rough with her and told her they didn't have time to be running to all these rooms. The treatment of my my mom was very disappointing. She was in extreme pain and would have to wait forever for her pain medicine. When they brought her pills in the morning, they wanted her to take them all in a handful, because they didn't have time to wait while she took them.Weekend staff seemed the worst.Anytime we complained something was done, but we had to call and complain about it.Then when it was time for her to come home, the social worker gave my brother a list and said he had to arrange everything on his own. No help at all. Worthless.Would not send anyone I cared for here. Or for that matter anyone at all.Unless I saw that extreme changes were made.
Joe Campbell: This is the nicest facility I have ever seen. It is so clean and the staff is so nice, especially Francine the Director of Nursing. Therapy staff is absolutely top notch! Highly recommend!!
Amanda Lorronnie: Very wonderful staff took great care of our uncle. Can not say enough good things about the whole environment/experience. The nursing was incredible! He was very pleased with it as well. I would recommend to anyone needing rehab.
Chandra Hutchinson: My mother ended up getting bed sores. Clean, nice and sanitary place; however, their services received from the staff/nurses were not great -there were things I was dissatisfied with. They do not have the patience to work with patients that have communication problems such as those who have had strokes and brain surgeries which was why my mother needed assistance and rehabilitation therapy. She had fallen out of her chair twice and was left halfway hanging out of her mobile chair. My mother has very little self mobility and requires alot of assistance, if you're looking for that kind of help this is not the right place.
Francine Reardon: This is a wonderful facility. It's beautiful, has spacious private rooms, and the employees are ALL very accommodating. The food is great, the care is timely, and the staff is beyond friendly. I would highly recommend this facility. There's no place like it!
Lisa Wilson: I work for a specialist office. I tried to call Sunterra five times today. My call was disconnect four times prior to being answered on the fifth call. I requested the billing office. I have been on hold for 15 minutes with no follow-up but I am afraid to hang up.
Susan Flint: Sunterra Springs in Independence, Missouri, definitely gets 5 stars from me! In choosing a place for a dear friend to rehab after a hospital stay, Sunterra Springs, came highly recommended. The staff was excellent, as well as the facility. My friend had rehabbed in other locations, and this was the first time he said, “They’re spoiling me here!” The PT and OT staff were kind, caring and had him getting stronger everyday. I had several occasions to interface with Cindy, the social worker on staff. She always returned my calls in a timely manner, and kept me apprised of what the game plan was. She went above and beyond in keeping me in the loop and really listened to me! She was just the best! If rehab was ever needed in the future, we would look no farther. EXCELLENT CARE! Thank you to everyone!
Julie Elliott: Lovely rooms and facility. PT was fabulous to my mom and helped tremendously! Food was inedible described by mom on soups and casseroles. Breakfast was good. Social worker was no help when I would call to inquire about what she needed. She would say I don’t know call PT or call the nurses. This would be right after a meeting they had and she was the topic. She was not pleasant to deal with. We were disappointed for the facility after being so excited to get in this new facility!
Brian Berger: So far I am very disappointed in the care here. The nurse's are few and far between. 1 nurse per 18 patient. What the hell. I was told that the nurse's only had about 4 to 5 people to care for at one time. We have only been here since Friday August 2nd. It is now Monday. They have ran out of meds, the have lost the med cart keys. It takes about average 15 to 30 minutes for call button to be answered. Although the staff is nice there are not enough for the proper care of the patient they are caring for. I have spoken to the nurse director 2 times already. It doesn't seem to make a difference. It a beautiful facility, great physical therapy room. But that doesn't matter if the care is subpar.
Sondra Shrake: Very nice facility and very well organized and professionally ran
Cara Haag: My mother recently spent a week here. Although she didn't want to be anywhere but home, her stay here was the next best thing. Unfortunately, she had to be there over her birthday. They had the whole dining room sing happy birthday to her and one of the nurse's ades went outside and picked her a bouquet of flowers for her room. I thought that was so kind and above and beyond! Everyone was kind and informative whenever I had any questions. We have been to a few rehab facilities, and they can be a nightmare but this one was great
Sara T: One star is one too many. Well I give that and a few more to the PT and OT, they were great. I dreaded nights and weekends. I spent many hours in tears wondering why they could be so mean and why they are nurses if they don’t care about people. They have no communication skills. If I ask for meds they simply didn’t return instead of telling me they ran out. At times the aide would simply turn off light and walk out at night. Literally ignoring me. Looking straight ahead and walk out. Which wow, starting the early morning of 7-4 about 1 am I had one dose of pain meds. That was the beginning of a two day nightmare. They didn’t tell me they ran out at that point but I had an idea since they only gave me two instead of 3. Even though they ran out I had another dose somehow in the morning again 2 tho instead of 3. He said he didn’t have access? Then after 3 hours requesting meds and nausea pills he finally walked in and said we don’t have either we have been waiting for them. I don’t know why they wouldn’t give me my other PRN ones too when I asked, my muscle relaxer. Shoot Tylenol was a nightmare to ask for. My dishcharge was delayed a few hours even waiting for them. I wasn’t comfortable leaving without what they said they ordered. Not sure how insurance would handle it if I left and had another dr prescribe again. My daughter reminded me of a night my heat got turned up to 90! This was the beginning of my stay, I couldn’t get up to change it or anything in my own at that point, oh my. Yet I was told I should not fear reporting something, yet once I did my care declined rapidly. Well not pleasemt before at least they treated my pain as prescribed. But not without rude comments and dirty looks. In fact, one time my dinner was not served to me.I requested it again. When the aide brought in a new one with grilled cheese and soup. The soup spilled on the sandwiches . Not edible in my opinion. When I complained she said “what do u want me to do about it!” I said hey, you tell me what you would do? Hungry forgotten then given soggy soaked sandwiches, she pooped off with “I would walk down And ask why! “ Yeah so although I had a lot of pain in my 2 week old knee I did that. When the kitchen staff came out with new plate, for some reason they couldn’t stop laughing, hard. I didn’t eat the food. Ok that’s one example.Going 12-13 hours with none of my PRN dr ordered pain meds, was awful. In fact, they simply didn’t give me any meds I requested that whole last day except Tylenol once. They did not care or show any compassion towards me. Day staff was better at explaining but, but only after several hours and requests.I have 3 more joints that need replacement. I won’t go back. Now I am scared to have them done at all.At night, common to have only 1 nurse for entire facilityI felt like Worse than 2nd class citizen. Luckily almost all situations are documented writing them down. I was told I can’t video cuz of hippa but I have seen hospital staff videos so I am not sure that’s true
Cynthia Fisher: Sunterra Springs is a beautiful facility, much like a lodge or spa. The accommodations are 5 star. The ground crew - nurses, nurses aides, OT/PT therapists and housekeeping are all highly skilled. The dining is the best. From the Director of Food Services on down this service is not matched in the Metro area. Meals are beyond excellent. However the administration from the Director down are seriously lacking in leadership skills, business acumen and professional conduct. In my humble opinion there is a great lack of transparency, honesty, communication, helpfulness, consideration, follow up and overall caring and compassion.Personally we were blind sided due to purposeful lack of communication, withheld information, double talk, and an abundance of smoke and mirrors. Be very wary of intentional lack of follow through following the initial (and only) meeting. We are pursuing legal counsel in these matters.I would not recommend this facility to my worst enemy.

Polaris Health and Wellness of Carmel Hills: what do users think?
Christophe Roberson: The kitchen manager there is rude and send staff home that he doesn't like and half cook food getting peoples sick wit other staff stilling frome the kitchen is sad
Kathy Dominguez: I will continue to put my review in on how y'all neglected my mom and almost killed her. Don't put anyone here they don't give a rats axx
Atlanta Nicole: Trashy employees with trashy attitudes. I wouldn’t leave anyone I know and love with these kinds of people. Everyone has an attitude and nobody ever wants to do their job but LOVE TO TELL OTHERS HOW TO DO THEIRS LOL. Please mind your business next time someone comes to your establishment if it does not concern you at all.
Rhonda Harris: I had a very good experience at Carmel Hills. My sister is a new resident there. All the questions and request I ever have are answered quickly by the care team. The room is kept very clean, and I really like that. My sister does not like to take her meds, so the care attendant crushes them up and puts them in applesauce to make sure she gets her daily meds. I would recommend this center.
Jen Zades: My Father spent several months at Carmel Hills from Sept of 22-Jan of 2023. The staff became like family to us. My favorite part of my visit was check in. Andrea was always so friendly. She knew our names after the first visit. She always asked how my Dad was doing. By the time my Dad passed away she knew my life story. It was always hard going to see my dad because of his failing health, however I was looked forward to my visit with her. Sara, Jessica and Audrey were also awesome. They loved my Dad and he loved them.
Rickie Moore: Awesome place and the staff OMG they are the best ever 😘😘😘😘
John Fry: Went to see Mother in law
Sheree Hoy: Great place to place your love ones. The staff are friendly and caring towards the residents are helpful to the visitors. The residents seem pleased with the services that they are receiving.
Brian Deconink: The visit was good.
Theodore Johnston: Clean, Kind, and very helpful.Thank you
Heather Stanton: Horribly rude staff. When someone actually does answer the phone, they’re incredibly hateful. I feel sorry for any patient that has to live here.
Deanna Mead: Would not recommend this place. They pretend to care and listen to your concerns, but don’t let that fool you. There was even a meeting we had, with 10 to 12 people present. Oh, they listened and pretended to care. They acted like they were hearing my complaints for the first time. They said things would get better and wanted me to keep my father there. But obviously it didn’t change because I removed my father from that neglectful place. Also, I spoke with other residents who were so upset, a few even cried because things were so upset. There is no compassion or empathy. One of the nurses even admitted to me that it was neglectful behavior. The communication between the staff was absolutely horrible. So very sad and I feel so bad for the residents that are stuck there with no other options. Don’t let the fake smilesfool you.
Justina Holzbaur: My grandma is Diana degette
Gaming Faith: This place hates freedom and the constitution DONT TRUST THEM WITH YOUR FAMILY
Jennifer Norton: My mom is in the Sunset Unit. The staff are much better since Amber has taken over. I feel like they are more attentive and care. Amber is amazing!! So thankful for her.The outside of the unit needs to be cleaned up. Flower gardens are full of weeds. Trash is around the building. This is an easy thing to take care of that makes the building look more out together and cared for.
Kelly kenney: The facility is older, nothing fancy. The staff though is very nice and has been a tremendous help to my mother. It's the people that have made my mother's stay go so smoothly in rehab.
Mari Cat: Awesome place! I was there for about a month and when it was time to go home it felt like I was leaving my family to go home to my other family.
Heidi Spear: My relative ran out of her antidepressant while she was here for rehab and they did not care one bit. She has went through more than one serious issue since she has been there. I cannot believe that they can get away with treating people that way. You cannot just stop ANTIDEPRESSANTS like that it is very dangerous!!!!
Renee Joy: Every part of the facility that I've been in has been clean. The staff have all been pleasant, and have listened if I've had a question or concern. The rehab patient whom I visit has made significant improvement in her physical strength during the past two weeks. She has said that there's a variety of food in her meals. There is a sign on the dining room wall listing several main course options that are always available (e.g., a hamburger).
Our Home: This place is so friendly and welcoming. Kim at the front desk always greets us with a smile. Even through her mask we can see her smile. The lobby area always smells so good. Not what one would expect from your typical nursing home. The nurses that take care of my dad are all so caring and considerate. Laura goes above and beyond. My only complaint is that my father says he sees his nurses sit down and eat dinner before he is given his and by the time he does get his food its cold.
Bill Bryan: I have been here since the ninth of March. Staff is excellent, physical therapist are great , very comfortable. The only issue I had is the food ,most of the time it's cold . Being a former cook the kitchen staff sucks! I even said to the dietitian if you wouldn't eat it don't serve it.
